The Funding process

Find funding streams available in your state.

Depending on where you live there may be many different types of funding available for your Smartbox device. Find your State and we will let you know what is currently available. If you are unsure at which is best for you, please get in touch and we can help find the best method of funding your device.

Download and complete your funding packet

On your state page, you can begin your application by filling out the short online form and receiving your funding packet in PDF format.

Meet with your Speech and Language Pathologist (SLP), who will determine which Smartbox device best meets your needs. Your SLP will then complete the evaluation report.

Forward a copy of your evaluation report to your physician. Your physician will need to complete a prescription and in some cases provide evidence of a face to face visit. Your prescription may be accompanied by further guidance and instructions.

Complete the remaining forms in the funding packet.

Send your funding packet to Smartbox for approval

Submit your funding packet to Smartbox by email, mail or fax. We will review the information you have supplied and submit your funding request to your funding source.

Once accepted, Smartbox will order your device and one of our local dealers will deliver it, alongside any other necessary support and training, to you.
